Navigating Your Plan with Lifestage

After your planning session, you'll receive your National Disability Insurance Scheme (NDIS) Plan, which will be constructed based on the discussions during your meeting. This plan holds a wealth of information, and we at Lifestage are here to help you understand it.

Your Personal Details

The first segment of your plan contains information about you, including details about your disability, daily activities, living situation, and the people who provide care for you.

Your Support Network

This segment is dedicated to the people who aid you voluntarily. These individuals might include friends, parents, siblings, or others who care about you and help you achieve your goals.

Community Services and Groups

Depending on your location, there might be community groups or services available to you. These can be public transport, health centers, schools, youth clubs, religious groups, sporting organizations, or libraries. These services, usually funded by the government, are open to everyone.

Your Objectives

This section focuses on your goals, which are what you hope to achieve with your NDIS plan. These goals are crucial for crafting an effective NDIS plan and can be either short-term (for example, learning photography, volunteering for a community group, visiting the library weekly, or joining a hydrotherapy class) or long-term (for instance, securing a job, building confidence to venture out with a support worker, moving out independently, learning to drive, or learning to cook).

Your Funded Supports

This part of your plan discusses your funding, the financial support you will receive from NDIS. Your funding will be divided into ‘support categories,’ which include 'Core Supports,' 'Capacity Building Supports,' and 'Capital Supports.' The amount of funding you receive in each category will depend on your specific needs, goals, and disability. Let's delve into each category:

1. Core Supports:

These are designed to assist with daily life and can cover help with daily tasks, essential items or technology you might need, and transportation.

2. Capacity Building Supports:

These are aimed at helping you become more independent. Each category under Capacity Building Supports has specific funding that must be used appropriately.

  - Support Coordination: A coordinator can help you navigate your plan and use your funding efficiently.
  - Improved living arrangements: This includes help with housing if you're looking to find a new place to live.
  - Increased social and community participation: You may receive support to participate more in your community.
  - Finding and keeping a job: This section provides assistance for job search and employment stability.
  - Improved relationships: Funding here may be used for learning how to understand and relate better with people.
  - Improved health and wellbeing: This is focused on improving your overall health but does not cover gym memberships.
  - Improved learning: This funding can help with education-related goals.
  - Improved life choices: This includes funding for plan management to help you manage your plan better.
  - Improved daily living: This can assist with improving independence, and the uses will depend on your specific goals.

3. Capital Supports:

This category covers higher cost items that can improve your independence. It might include funding for a wheelchair, modifications to your home or vehicle, or other similar needs.
